General Complaints
In accordance with the State Comptroller's Guidelines for Audit Committee Charters, Tennessee Tech Internal Audit provides the following form for complaints about accounting, internal control, auditing matters, Code of Conduct violations, and other related issues.
This form can be submitted online directly to Internal Audit or can be printed, completed, and mailed to Internal Audit. The complainant will remain completely anonymous to Internal Audit unless he/she completes the last section of the form, which contains identifying information.
Complaints about accounting, internal control, auditing matters, code of conduct, and other related issues may be investigated by Tennessee Tech Internal Audit or may be referred to another, more appropriate unit for follow-up. Complaints about discrimination, harassment, or retaliation will be referred to the Compliance Office for investigation.
